Most appointments with a provider are short, and many of the things that matter most — pain levels, daily function, side effects, sleep, mood — don’t always get fully captured in a brief office visit.

By completing short digital check-ins, you provide your care team with a more complete and accurate picture of your health. This helps your provider move beyond guesswork and base decisions on what you’re experiencing over time, not just what comes up in a single conversation.

In pain management, substance use disorder, and other long-term treatment settings, providers are required to document their decisions carefully. When information is missing or unclear, it can slow things down or lead to more cautious treatment than necessary.

What You’re Being Asked To Do

If you’re being treated for a condition that may require the use of a controlled substance, your doctor could decide to enroll you in our Perspectives in Care program. You’ll be asked to complete brief digital questionnaires, sometimes before each visit or throughout the month. These take only a few minutes and help your provider fully understand how pain, symptoms, sleep, mood, and medications are affecting your daily life.

Your responses are organized into clear summaries for your care team, helping them see what’s improving, what isn’t, and where attention is needed.
